Cemetery/Crematorium Representative Activity Reporting Form 2017/2018
NSW Cemeteries and Crematoria Activity Reporting
Uniting Resources on behalf of Cemetery and Crematoria NSW (CCNSW) is collecting annual activity data for all Uniting Church cemetery and crematoria facilities in NSW.
While CCNSW only covers NSW, Uniting Resources is also requesting that Uniting Church cemetery and crematoria facilities in the ACT also provide the same information to Uniting Resources so as to ensure Property Trust records are up-to-date.
Part 4 of the Uniting Church in Australia Regulations defines “Responsible body” as meaning a body that is responsible for the management and administration of property, and which either itself carries out those responsibilities or appoints another body to undertake them either in whole or in part.
Uniting Resources therefore asks that a “nominated representative” be appointed from each Responsible body that is responsible for the management and administration of cemetery and crematoria facilities in NSW and the ACT to complete the attached annual activity reporting form for each cemetery and crematorium they manage by the 24 August 2018. This includes all facilities able to undertake future interments or cremations. If more than one cemetery is managed we request that the form is filled out for each individual cemetery.
NSW Cemeteries and Crematoria Register
The NSW Cemeteries and Crematoria Register is maintained by CCNSW in accordance with Section 27 of the Cemeteries and Crematoria Act 2013. A public version of the Register is accessible here.
The Act requires operators to notify CCNSW of any material change in their registered information within 14 days after the change occurs.
Responsibilities of the nominated representative
The nominated representative is responsible for the administration and management of plot and niche purchases; complying with statutory obligations for reporting; and all works carried out in the cemetery including overall maintenance of the grounds, managing contractors and volunteers.
Please complete one form for each operational facility. An operational facility includes facilities able to undertake future interments or cremations
